CONSENT || RULES || LIABILITY
Tattooing is carried out under hygienic conditions with suitable sterile instruments and safe techniques and in accordance with EN 17169 and corresponding national requirements.
The studio/tattooist/practitioner confirms that the client has been offered all possible information and that the client has been advised to consider all information carefully. All guidelines drawn up by the GGD can be read on our or their website.
The studio / artist / practitioner is not liable for any complications or risks.
The studio / artist / practitioner cannot be held liable for the outcome of the tattoo / PMU or similar treatment
Getting a tattoo / PMU is entirely at your own risk and the client has been sufficiently informed of all associated risks and complications. All information can be found on our website, as well as in the consent form, the information available in the studio and on the GGD website.
The client gives 100% permission to have his/her body touched by the artist/practitioner.
The client has been informed of the presence of an assistant whom he or she can go to regarding matters such as 'over-separating' behavior.
All photos taken of the client can be used for marketing purposes.

Het tatoeëren wordt uitgevoerd onder hygiënische omstandigheden met geschikte steriele instrumenten en veilige technieken en volgens EN 17169 en overeenkomstige nationale eisen .
De studio / tattooeerder / behandelaar bevestigd dat aan de cliënt alle mogelijke informatie is aangeboden en dat de cliënt het advies heeft gekregen om alle informatie zorgvuldig te overwegen. Op deze website kunnen alle richtlijnen welke door de GGD zijn opgesteld nagelezen worden.
De studio / artiest / behandelaar is niet aansprakelijk is voor eventuele complicaties of risico's.
De studio / artiest / behandelaar kan niet aansprakelijk gesteld worden voor de uitkomst van de tattoeage / pmu of soortgelijke behandeling
Het laten zetten van een tattoeage / pmu is geheel op eigen risico en de client is voldoende op de hoogte gesteld van alle bijhorende risico's en complicaties. Zowel is alle informatie op onze website terug te vinden, als in het toestemmingsformulier, de informatie aanwezig in de studio en op de webiste van de GGD.
De cliënt geeft 100% toestemming om zijn/haar lichaam aan te laten raken door artiest / behandelaar.
De cliënt is op de hoogte gesteld van de aanwezigheid van een assistente waar hij of zij terecht kan aangaande zaken zoals 'over-scheidend' gedrag.
Alle foto's die welke worden genomen van de cliënte kunnen gebruikt worden voor marketing doeleinden.
